Home Care Connection

A variety of community and outpatient services are provided by the Home Care Connection Staff. Home Care Connection is a hospital Based Public Health, Certified Home Health, Hospice, and Home Care Aide Agency.

Home Care

Home Care is coordinated with your physician and includes:

  • Skilled nursing with registered nurses who provide health care in your home.
  • Special treatments, injections, wound care, diabetic teaching, medication management and IV therapy.
  • Supervision of home health aide services ordered by your physician.
  • Physical therapy, Occupational therapy, and Speech therapy
  • Case management of the frail elderly

Home Care Aide Services

  • Assistance with personal care and grooming
  • Routine light housekeeping
  • Plan, prepare and service meals
  • Grocery shopping for food and essentials
  • Teach household routines and skills

Hospice

A special way of caring for people with terminal illnesses and their families is available through Hospice. Hospice treats the whole person as well as offering support to the family. Services are coordinated by a team of professionals (medical director, nurse, social worker, and clergy) and may include:

  • Physician visits
  • Nursing visits
  • Home Health Aide care
  • Social worker services
  • Dietary counseling
  • Medical equipment and supplies
  • Medications for symptom control and pain relief
  • Physical, Occupational and Speech Therapies
  • Volunteers

Corporate/Community Health

A full range of services are available to all members of the community and rural areas. They are tailored to be of particular benefit to the businesses, offering employers the opportunity to provide on site health screenings and wellness education to their employees at special group rates. Families Forward

Families Forward is a parent support program available to pregnant women and their families with children newborn to five years old living in Humboldt County. This voluntary program is tailored to meet each family’s individual goals through home visits. The focus of the visits relate to child parent interactions, child growth and development, and assisting families to find resources in the community.

Immunizations

Scheduled immunization clinics are held on a regular basis offering immunizations by appointment. Childhood and adult immunizations are offered. Contact the office for complete information on specific shots available and clinic times. Influenza immunization clinics are offered in the fall

WIC and Maternal Health

WIC is a special Supplemental Nutrition Program for Women, Infants, and Children. It serves to safeguard the health of low-income women, infants, & children up to age 5 who are at nutritional risk by providing nutritious foods to supplement diets, information on healthy eating, and referrals to home care. The clinic is held two days a month in Humboldt and appointments can be made by calling the office.

Case Management of the Frail Elderly

A process to optimize the self-care of individuals and their families to enable them to remain independent in their own home.

Other

Girl Talk/Guy Talk: pregnancy prevention program offered to 5th and 6th graders in the county.

Child Care Nurse Consultant: works with childcare providers to improve childcare quality.

Bio-emergency preparedness: working with local agencies to plan for future public health emergencies which may include pandemic influenza.
